The health plan that sent it to me said it was for $10. When I went to use it at Panera Bread it only had 91 cents on it. I had to make an account just to find out the balance on this card, and then verify with a code sent to my email address. So here the security was great.
My question is, did they only put 91 cents on it, did it lose value since I received it in December, or had someone already used it for $9.09 and then sent it to me?
What do you think?

I would contact your health plan and see what they say about it. If nothing else, maybe they will send you a new one. Can’t hurt to try.

My generic thought: I’ve had $5 – $20 gift cards to places like Panera, DQ, etc given to me by Realtors, shop owners, random others and more than once I’ve had a card or two come up as invalid or $0 balance or etc. The killer for me is that the only suggestion to go further is to (like you said) go through the extra steps of making an account then going through their steps. All of which used up my own time for a net result being the same – an unusable gift card. So whenever I present a gift card, I make sure I have a back up form of payment at the ready if it’s something I really needed to get anyway.
Dumb. But there’s the long of it!
